Фізична терапія дітей з бронхіальною астмою ІІ ступеня тяжкості на стаціонарному етапі

Abstract

UA : Кваліфікаційна робота: 70 стор., 7 табл., 1 рис., 72 літературних джерела. Об’єкт дослідження – функціональний стан дихальної системи дітей з бронхіальною астмою середнього ступеня важкості. Мета роботи – дати оцінку ефективності застосування сучасних методів фізичної терапії в реабілітації дітей з бронхіальною астмою II ступеня тяжкості на стаціонарному етапі. Методи дослідження – аналіз та узагальнення літературних джерел; аналіз історій хвороби; опитування, традиційні та розрахункові методи визначення основних показників системи зовнішнього дихання відповідно до Набору базових категорій МКФ-ДП, які характерні для обструктивних захворювань легень; методи математичної статистики. На початку дослідження показано, що в дітей, хворих на бронхіальну астму, спостерігається зниження показників функції зовнішнього дихання та якості життя за доменами МКФ-ДП. Відхилення фактичних показників від належних склало: ЖЄЛ – 27 %, ФЖЄЛ – 38 %, ОФВ1 – 44 %, Nвид – 33 % відповідно; індекс Тіффно – 79 %, Твид – 23 с. Розширення рухового режиму та застосування респіраторної гімнастики сприяло приросту показників функції зовнішнього дихання в основній групі в порівнянні з контрольною. Застосування засобів фізичної терапії у реабілітації та своєчасна корекція рухового режиму дозволили покращити бронхіальну прохідність на рівні «структури», поліпшити функціонування кардіореспіраторної системи організму на рівні «функції», підвищити працездатність підростаючого покоління на рівні «активності», сприяти зниженню ступеня важкості захворювання, обмеженню застосування медикаментозної терапії, більш тривалій ремісії на рівні «участі».

The object of the study is the functional state of the respiratory system of children with bronchial asthma of moderate severity. The purpose of the work is to evaluate the effectiveness of the use of modern methods of physical therapy in the rehabilitation of children with bronchial asthma of the II degree of severity at the inpatient stage. Research methods – analysis and generalization of literary sources; analysis of medical histories; surveys, traditional and calculation methods for determining the main indicators of the external respiratory system in accordance with the Set of basic categories of the ICF-DP, which are characteristic of obstructive lung diseases; methods of mathematical statistics. At the beginning of the study, it was shown that in children with bronchial asthma, there is a decrease in indicators of the function of external breathing and quality of life according to the ICF-DP domains. The deviation of the actual indicators from the appropriate ones was: LVEF – 27%, FLVEF – 38%, FEV1 – 44%, Ntype – 33%, respectively; index Tiffno – 79%, Tweed – 23 p. The expansion of the motor regime and the use of respiratory gymnastics contributed to the increase in the indicators of the function of external breathing in the main group compared to the control group. The use of physical therapy in rehabilitation and timely correction of the motor regime made it possible to improve bronchial patency at the level of «structure», to improve the functioning of the cardiorespiratory system of the body at the level of «function», to increase the working capacity of the younger generation at the level of «activity», to help reduce the severity of the disease, to limit the use drug therapy, longer remission at the «participation» level.
