Особливості застосування засобів фізичної терапії для відновлення рухової функції верхньої кінцівки в пацієнтів у гострому періоді ішемічного інсульту

Abstract

UA : Об’єктом дослідження – функціональна активність верхньої кінцівки у ранньому відновному періоді осіб які перенесли ішемічний інсульт. Предмет дослідження – вплив сучасних засобів фізичної терапії для поліпшення рухової функції верхньої кінцівки у ранньому відновному періоді осіб які перенесли ішемічний інсульт. Мета дослідження – оцінка ефективності застосування засобів фізичної терапії в комплексній реабілітації для відновлення рухової функції верхньої кінцівки в пацієнтів у гострому періоді ішемічного інсульту. Методи дослідження: аналіз науково-методичної літератури; оцінка функціональної незалежності особи у побуті за шкалою FIM; оцінка рухової функції верхньої кінцівки за тестом ARAT; аналіз досягнення мети реабілітації за МКФ; методи математичної статистики. Застосована програма фізичної терапії для пацієнтів у гострому періоді ішемічного інсульту забезпечила поліпшення рухової функції верхньої кінцівки при достовірному позитивному впливі одночасно на два основні компоненти МКФ: функції організму (відновлення м’язової сили та моторного контролю) за тестом ARAT (р<0,05) і активність/участь (підвищення рівня самообслуговування, мобільності та соціального функціонування) за шкалою FIM (р<0,05). Доведено, що застосування функціональної фізичної терапії у складі комплексної реабілітаційної програми є ефективним і зручним у стаціонарних умовах. Це забезпечує можливість самостійного виконання вправ пацієнтами, сприяє підвищенню їх мотивації та активної участі у реабілітаційному процесі.

EN : The object of the study is the functional activity of the upper limb in the early recovery period of people who have suffered ischemic stroke. The subject of the study is the influence of modern physical therapy tools to improve the motor function of the upper limb in the early recovery period of people who have suffered ischemic stroke. The purpose of the study is to assess the effectiveness of the use of physical therapy tools in complex rehabilitation to restore the motor function of the upper limb in patients in the acute period of ischemic stroke. Research methods: literature review; assessment of the functional independence of a person in everyday life using the FIM scale; assessment of the motor function of the upper limb using the ARAT test; analysis of the achievement of the rehabilitation goal according to the ICF; methods of mathematical statistics. The applied physical therapy program for patients in the acute period of ischemic stroke provided improvement of the motor function of the upper limb with a significant positive effect simultaneously on two main components of the ICF: body functions (restoration of muscle strength and motor control) according to the ARAT test (p<0.05) and activity/participation (increasing the level of self-service, mobility and social functioning) according to the FIM scale (p<0.05). It has been proven that the use of functional physical therapy as part of a comprehensive rehabilitation program is effective and convenient in inpatient conditions. This provides the possibility of independent performance of exercises by patients, contributes to increasing their motivation and active participation in the rehabilitation process.
