Фізична терапія в системі відновлення функціональної незалежності дітей із геміплегічною формою церебрального паралічу

Abstract

UA : Об’єкт дослідження – процес фізичної терапії дітей із геміплегічною формою церебрального паралічу. Предмет дослідження – засоби та методи фізичної терапії, що забезпечують формування функціональної незалежності у дітей із геміплегічною формою церебрального паралічу. Мета дослідження – науково обґрунтувати та розробити програму фізичної терапії, спрямовану на підвищення рівня функціональної незалежності дітей із геміплегічною формою церебрального паралічу. Методи дослідження. Теоретико-аналітичні методи. Валідизовані клінічні методи оцінювання функціонального стану скелетно-м’язової системи за доменами МКФ-ДП: Goniometria articulorum, Modified Ashworth Scale, Gross Motor Function Classification System, Barthel Activities of Daily Living Index. Методи математичної статистики. Порівняльний аналіз ефективності реабілітаційних програм за сукупністю досліджуваних показників підтвердив переважну ефективність програми фізичної терапії. Більш виражена позитивна динаміка в основній групі зафіксована за показниками рухливості дистальних суглобів кінцівок, рівня спастичності м’язів-згиначів верхньої кінцівки, грубих рухових функцій (GMFM) та побутової самостійності (індекс Barthel ADL). Отримані результати підтверджують доцільність впровадження розробленої програми фізичної терапії у клінічну практику реабілітаційних відділень для дітей із геміплегічною формою церебрального паралічу.

EN : The object of the study is the process of physical therapy of children with hemiplegic form of cerebral palsy. The subject of the study is the means and methods of physical therapy that ensure the formation of functional independence in children with hemiplegic form of cerebral palsy. The purpose of the study is to scientifically substantiate and develop a physical therapy program aimed at increasing the level of functional independence of children with hemiplegic form of cerebral palsy. Research methods. Theoretical and analytical methods. Validated clinical methods for assessing the functional state of the musculoskeletal system according to the domains of the ICF-DP: Goniometria articulorum, Modified Ashworth Scale, Gross Motor Function Classification System, Barthel Activities of Daily Living Index. Methods of mathematical statistics. Comparative analysis of the effectiveness of rehabilitation programs according to the set of studied indicators confirmed the predominant effectiveness of the physical therapy program. More pronounced positive dynamics in the main group was recorded according to the indicators of mobility of distal joints of the extremities, the level of spasticity of the flexor muscles of the upper limb, gross motor functions (GMFM) and household independence (Barthel ADL index). The results obtained confirm the feasibility of implementing the developed physical therapy program in the clinical practice of rehabilitation departments for children with hemiplegic cerebral palsy.
