Клініко-біохімічні показники крові у осіб із відкритими черепно-мозковими травмами

Abstract

UA : Робота викладена на 75 сторінках друкованого тексту, містить 8 таблиць, та 12 рисунків у додатках. Список літератури включає 55 джерел. Об’єкт дослідження ‒ капілярна та венозна кров осіб із відкритими черепно-мозковими травмами. Мета роботи – дослідження клініко-біохімічних показників крові у хворих з відкритими черепно-мозковими травмами у динаміці лікування. Методи дослідження – клінічні, біохімічні і статистичні. У результаті дослідження встановлено, що при надходженні хворих до лікарні зростає вміст лейкоцитів за рахунок СЯН, глюкози і ШОЕ, показники стану нирок, лейкограми (за винятком СЯН) та електроліти знаходяться у межах норми, а показники кількості еритроцитів та гемоглобіну зменшуються. Після операції відбувається зниження вмісту гемоглобіну, еритроцитів, електролітів та усіх біохімічних показників крові, підвищення макро- і мікрофагів і ШОЕ. При подальшій інтенсивній терапії усі змінені показники крові, окрім ШОЕ, повертаються до норми. Новизна роботи – вперше проводиться дослідження клініко-біохімічних показників крові у хворих з відкритими черепно-мозковими травмами в екологічно несприятливих умовах м. Запоріжжя. Значимість роботи – результати дослідження поширюють уявлення про стан показників крові у хворих із відкритими черепно-мозковими травмами та у динаміці їх лікування, що дозволяє їх використовувати як додатковий діагностичний та прогностичний критерій при оцінюванні ступеня тяжкості черепно-мозкової травми. Отримані результати можуть бути використані медичними установами для більш прицільного проведення лікарської терапії.

The purpose of the study is to study the clinical and biochemical parameters of blood in patients with open traumatic brain injuries in the dynamics of treatment. Research methods are clinical, biochemical and statis The study found that when patients are admitted to the hospital increases the content of leukocytes due to segmented neutrophils, glucose and erythrocyte sedimentation rate, indicators of kidney status, leukograms (except segmented neutrophils) and electrolytes are within the normal range, and indicators of red blood cells and hemoglobin decrease. After surgery there is a decrease in the content of hemoglobin, erythrocytes, electrolytes and all biochemical parameters of blood, increase of macro- and microphages and erythrocyte sedimentation rate. With further intensive care, all altered blood counts, except erythrocyte sedimentation rate, return to normal. The novelty of the work is the first study of clinical and biochemical parameters of blood in patients with open craniocerebral traumas in ecologically unfavorable conditions of Zaporozhye. Significance of the work - the results of the study extend the understanding of the status of blood indices in patients with open traumatic brain injuries and in the dynamics of their treatment, which allows them to be used as an additional diagnostic and prognostic criterion in assessing the severity of traumatic brain injury. The results obtained can be used by medical institutions for more appropriate drug therapy.
