Abstract
UA : Об’єкт дослідження: функціональний стан військових, які отримали бойові травми та/або мають діагноз посттравматичного стресового розладу.
Предмет дослідження: зміст і структура комплексної програми фізичної реабілітації військових із використанням кардіореспіраторних тренувань.
Мета дослідження: оцінка ефективності програми кардіореспіраторних тренувань у складі комплексної реабілітації військовослужбовців із посттравматичними стресовими розладами щодо показників функціонування організму, активності та участі відповідно до МКФ порівняно зі стандартною реабілітацією.
Методи дослідження: теоретичний аналіз і узагальнення наукової та методичної літератури; педагогічне спостереження; стандартизоване тестування (6MWT, PCL-5, PSQI, PHQ-9, WHODAS 2.0); МКФ; методи математичної статистики.
Програма кардіореспіраторних тренувань в комплексній реабілітації забезпечила клінічно значуще покращення проходження дистанції 6MWT на 85 м (p<0,001) і психоемоційних функцій за PCL-5 (на 14,4 пункти, p<0,001); зниження депресивної симптоматики (PHQ-9 –5,3 балу); покращення дихальних функцій FVC на 9,2 % (p<0,01) та підвищення SpO₂ під навантаженням на 2,1 % (p<0,05); відбулась нормалізація сну (PSQI –5,3 балу, p<0,001) і загальний бал WHODAS 2.0 знизився на 35,5 % (p<0,001) через аеробні адаптації. Зведений МКФ-профіль фіксує покращення за всіма кодами МКФ, що підтверджує системний характер реабілітаційного ефекту.
EN : Object of the study: functional state of military personnel who have received combat injuries and/or are diagnosed with post-traumatic stress disorder. Subject of the study: content and structure of a comprehensive program of physical rehabilitation of military personnel using cardiorespiratory training. Purpose of the study: assessment of the effectiveness of the cardiorespiratory training program as part of the comprehensive rehabilitation of military personnel with post-traumatic stress disorders in terms of indicators of body functioning, activity and participation according to the ICF in comparison with standard rehabilitation. Research methods: theoretical analysis and generalization of scientific and methodological literature; pedagogical observation; standardized testing (6MWT, PCL-5, PSQI, PHQ-9, WHODAS 2.0); ICF; methods of mathematical statistics. The cardiorespiratory training program in complex rehabilitation provided a clinically significant improvement in the 6MWT 85 m distance (p<0.001) and psychoemotional functions according to PCL-5 (by 14.4 points, p<0.001); a decrease in depressive symptoms (PHQ-9 -5.3 points); an improvement in respiratory functions FVC by 9.2% (p<0.01) and an increase in SpO₂ under load by 2.1% (p<0.05); normalization of sleep occurred (PSQI -5.3 points, p<0.001) and the total WHODAS 2.0 score decreased by 35.5% (p<0.001) due to aerobic adaptations. The combined ICF profile records improvements across all ICF codes, confirming the systemic nature of the rehabilitation effect.
